Several states require state specific forms be signed by an individual when accessing MVR or criminal records. This page provides links to those state specific forms.

Please click on the appropriate link below to access a state specific form. Print the form and have the applicant complete and sign the form. Please forward, via fax, the completed form to Background Profiles at (925) 974-1840. If the state you are looking for is not listed, the standard Background Profiles release form, dated 01/10/03 will be acceptable.
