If you need to locate a collection center nearest to your location, you
may do so by clicking on the appropriate logo below. Please be sure to
select the logo for the testing lab that you have Chain of Custody forms
for. Selecting the wrong lab may result in "Out of Network" charges.
Be sure to select
"Drug Screen Collection" from the LIST
under
"Search Specifications".
